Help Window Cleaning Paper Slips?

So I just got a chain restaurant as a client recently, and he had a window cleaner for another store close by, but he preferred my services and wanted me to do the entire chain in my area. But the one thing that he liked that the other guy did that I never thought of doing, which is actually a good idea, was to give him a paper slip every time he went and did the cleaning. It wasn't necessarily a payment slip, it was just half a piece of paper with the name of the company, date, and stated the services that where done. In the clients words, it was just something that the window cleaner would give him at the end of the cleaning to let him know that he was there since the client has no time to be checking in to see if he came to clean the windows or not.

Do any of you guys do this sort of thing? If so, can you guys post a picture of it so I can get some ideas? It would be greatly appreciated. Thanks for all your help.
